Intermittent Skilled Nursing
We provide wound care, intravenous, and nutrition therapy, injections, medication management, and patient/caregiver training. We will oversee your overall health which may include:
- Wound care for pressure sores or a surgical wound
- Patient and caregiver education
- Intravenous or nutrition therapy
- Injections
- Monitoring serious illness and unstable health status
Physical Therapy
We provide home safety evaluation, work closely with our patients to increase strength and mobility, daily activity tolerance, reinforce the use of medical devices, and reduce unnecessary falls in your home. Physical Therapists can put together a plan of care to help a patient regain or strengthen, use of muscles and joints which may include:
- Gait and balance training and strengthening
- Home safety evaluation/fall prevention
- Patient and caregiver training to optimize patient independence
Occupational Therapy
We help patients achieve independence in their homes. Our therapist will work with you to regain your ability to carry out your daily activities, which may include:
- Assess home environment for adaptive equipment
- Training for energy conservation and physical development
- Focuses on improving activities related to eating, bathing and dressing
Speech Therapy
The ability to communicate is important at any age. Our speech therapist will help you relearn how to communicate, which may include:
- Assist patients in improving speech after injury or illness
- Help patients with imparied speech to regain the ability to communicate clearly
- Treat swallowing and cognitive issues
Medical Social Services
Medical social workers provide various services to the patient including:
- Short-term Counseling and facilitating referrals for longer-term counseling needs
- Locating community resources to help the patient with their recovery
- Coordination services for complex, medical conditions
- Assist in preparing advanced directives
Home Health Aide
We offer intermittent visits for hands-on care. Certified Home Health Aides can help the patient with their basic personal needs, which may include:
- Bathing and personal hygiene and dressing
- Walking, transfers, ambulation, exercises
- Other tasks delegated by the Registered Nurse
Healthcare treatments that were once offered only in a hospital or a doctor’s office are now available in your home. Home healthcare is less expensive, more convenient, and is just as effective. Our goal is to provide treatment for illnesses or injuries to help you or your loved ones get better, regain your independence, and be self-sufficient.
